I submit that my child is physically fit and able to participate in strenuous activity and hereby waive WE GOT NEXT BASKETBALL CLUB, WGNBC Administration/Coaches/Skills Workout Trainers and WGNBC facilities/gyms/schools, of all responsibility for illness or injury sustained. I hereby authorize WGNBC Administration and Coaches to act on my behalf in their best judgement in any emergency medical situation when I am not present, however, I understand that I am solely responsible for payment of any such medical expenses. I understand that this is just a pre-registration form and that upon receipt of my registration payment that the payment is non-refundable. I also understand that any player who does not abide by team rules or regulations during the WGNBC Club Tryouts is subject to dismissal without refund or recourse.
